JOB TITLE: Social Worker

MAIN FUNCTION: The role of the social worker in a long-term care facility is to enable each individual to function at the highest possible level of social and emotional wellness

REPORTS TO: Director of Social Services

DUTEIS AND RESPONSIBILITIES:

• Provides emotional support to residents and family.
• Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological well-being of each resident.
• Assesses the social, emotional and financial needs of the resident and/or family.
• Documents periodic evaluations on social and family status of each resident.
• Assists residents and families in understanding services offered in the facility.
• Participate in multidisciplinary team conferences.
• Develops discharge plans for residents; follow-up visits and supportive casework may be made.
• Assists residents and families explore and utilize appropriate community resources.
• Prepares and keeps a record of social service contacts made with residents.
• Acts as liaison for Resident Council meetings.
• Assists residents with their mail if they so request or are unable to handle it themselves.
• Acts as a referral agent to community agencies as needed
• Other duties as requested by the M.S.W. consultant or the Director of Social Services.

EDUCATION: Bachelor degree in a field related to social work, or related field

EXPERIENCE: Educational degree B.S. or four (4) years social work experience, or equivalent combination of education and experience.

CONTACTS: Daily contact with patients/residents and the professional and non-professional members of the community.

WORKING CONDITIONS: Well-ventilated, well-lighted surroundings, congenial atmosphere.

PHYSICAL DEMANDS: Considerable walking and standing, sitting.

JOB HAZARDS: Contacts with patients on nursing units with hazard of infection.

POSSIBLE ADVANCEMENT: With appropriate additional formal education to promotion.

SAFETY: Accountable for implementing assigned responsibilities and activities to attain safety goals and objectives by complying with applicable rules and regulations and performing tasks and supervising others in performing tasks in a safe manner.
